	Financial Hardship Application Form, Instructions and Policy
	Intended Use Plan (IUP) Project Priority Readiness List  An IUP is developed annually for each federal fiscal year. The IUP describes the DWSRF, program administration, project scoring, and incorporates public comment. Lists of projects which qualify for subsidized or unsubsidized financing within the coming fiscal year or in future years are provided. The most recent DWSRF IUP Readiness List only will be sent if this box is checked. DOH will not publish the entire IUP document; however, you may print a copy from the DOH's DWSRF web site.
